Promesa Behavioral Health is a private non-profit social services agency serving the community for over 35 years. We are excited to announce we are implementing a new program for Enhanced Intensive Services Foster Care (EISFC).
This program will provide support and guidance to youth in foster care who may benefit from additional services and mental health supports that traditional foster care may not provide. The goal is for the youth to gain assistance with their unmet complex care needs, while residing with a Specialized Resource Parent who provides stability while they build a bridge to a more permanent home.
We are currently seeking qualified individuals for EE/EISFC Case Manager positions( 1-3 Positions)
Job Summary
The EE/EISFC Case Manager's role is to assist in the child's transition into a family, focusing on the child, the Specialized Caregiver (i.e., resource or foster parent), the biological parent when appropriate and the siblings when feasible. The Case Manager will work with the entire family and can help identify underlying triggers to trauma and assist in developing strategies to mitigate or resolve stressors which will assist with developing and carrying out the mental health assessment. The Case Manager can identify and work with the members of the treatment team to resolve, reduce and mitigate further trauma to youth by empowering youth and teaching coping skills.
Typical duties may include, but is not limited to the following:
- At the beginning of an EE/EISFC placement, the supervisor, social worker, and EE/EISFC Case Manager will meet with the family to review all the applicable policies.
- Work closely with treatment team to ensure proper delivery of EE/EISFC services to youth according to their plan of care.
- Meet with the child/non- minor and the Specialized Caregiver as often during the week as necessary to meet contracted hours for the level assigned and to monitor progress as per the plan of care.
- Will be required to respond to Specialized Caregiver home for escalated situations, crisis, or any heightened incident requiring additional supports, per timeframes and hours outlined in Fresno County contract #24-500.
- Will provide specialty mental health services and bill Medi-Cal.
- Ongoing, weekly communication with the Promesa EE/EISFC treatment team via face to face, phone calls, and email communication.
- Participate in all appropriate sessions and meetings in order to assist youth in their treatment.
- Attend monthly or quarterly "progress" meetings, with social worker and family.
- Work with the social worker and treatment team to update the plan of care, if needed.
- Participate in Child and Family Team meetings with EE/EISFC Team
- Additional duties as assigned
- Overnight stipends apply to those Case Managers who work overnight shifts and/or on call overnight shifts
- The Case Manager will work with the entire family and can help identify problems as well as develop strategies to mitigate or resolve problems
- A Plan of Care, including a mental health treatment plan will be developed according to needs of family and child/non-minor dependent within the designated timeframe required
- May gather information to assist with mental health assessment.
- Provide service to youth in accordance to their approved case plan: (Assessment, Plan of Care, Needs and Service)
- Will complete progress notes, and other required documentation in her EHR system for supervisory review and to assist treatment team to ensure proper service delivery of EE/EISFC services.
Minimum Requirements for the position:
A Bachelor's Degree from an accredited or state approved college in psychology, social work or human services.
