Job Summary
The EE/EIFSC Case Manager is part of the treatment team that works with Specialized Caregivers to support youth who are in out of home care. Will be responsible for working in collaboration with the clinician in the utilization of community resources to meet youth’s unique needs which be outside of the agency’s expertise. They work closely with treatment team to ensure proper delivery of EE/EISFC services to youth according to their plan of care. They possess specialized skills in working with children who have experienced complex trauma, who have emotional and behavioral difficulties as well as developmental delays.
The EE/EISFC Case Manager’s role is to assist in the child’s transition into a family, focusing on the child, the Specialized Caregiver (i.e., resource or foster parent), the biological parent when appropriate and the siblings when feasible. The Case Manager will work with the entire family and can help identify underlying triggers to trauma and assist in developing strategies to mitigate or resolve stressors which will assist with developing and carrying out the mental health assessment. The Case Manager can identify and work with the members of the treatment team to resolve, reduce and mitigate further trauma to youth by empowering youth and teaching coping skills.
The Case Manager can work one-on-one time with the child in the home as well as taking them out in the community as part of the treatment plan and for earned rewards can work in the home, and other settings where direct services are needed. They spend time with the family and child implementing the treatment plan and assisting the foster/resource parents in meeting treatment goals.
Promesa Behavioral Health is a private non-profit social services agency serving the community for over 30 years. We provide and operate eight STRTP (Short Term Residential Therapeutic Program) facilities, foster family, adoptions, and substance use treatment services.
We are currently seeking two Registered Mental Health Clinician (AMFT, APCC and/or ASW) for our STRTP youth, typically ages 11-17, who are referred through Juvenile Probation and Social Services.
Provides supervision, protection and care of the clients in a group home setting and in community setting.
